Management
Case management offers outreach services such as needs assessment, linkage with community services and monitoring. Referral sources include but are not limited to school personnel, juvenile justice system, the Alabama Department of Human Resources, other human service agencies and medical institutions, as well as other programs of AltaPointe Health Systems. |
