How to recognize an AltaPointe employee for exceptional customer service

	1.	Observe an AltaPointe staff member providing exceptional customer service.
	2.	Pick up an AltaPraise card or go to AltaPraise.org, complete form, drop in box, mail or return via Internet.
	3.	Five-Star CS volunteer collects cards, sends to PR Office for review. (Online forms go directly to PR Office.)
	4.	Supervisor verifies that staff member merits the AltaPraise and that staff member is in “good standing.”
	5.	AltaPraise cards/forms displayed on 5SCS Bulletin Boards, website.
	6.	Supervisor presents recognition pin to your AltaPointe Star. Stars recognized in AltaNews and AltaPraise Flyer.
	7.	AltaPraise Review Team selects “Star of the Quarter” from each quarter’s group of “Stars.”
	8.	Star of the Quarter receives special recognition.
	9.	AltaPraise Review Team, Administration, other appropriate staff, select “Super Star of the Year.”
	10.	"Super Star of the Year" named at annual event.
