 AltaPointe values employees who exhibit customer-focused behaviors and has established the AltaPraise Recognition program to show these employees how important they are to the organization. AltaPraise, which means “highest praise,” encourages consumers, family members, co-workers and all other customers to recognize an employee by submitting an AltaPraise card to the Five-Star team.
AltaPraise cards and collection boxes are located in all AltaPointe facilities. To access the AltaPraise electronic form, go to: AltaPraise Recognition Card. A completed card, or electronic form, must include a detailed description of an AltaPointe employee’s exceptional customer service performance.

Who may submit an AltaPraise card?
Anyone – staff members, consumers, consumer family members, other provider staff members, referral sources, and members of the public – may submit an AltaPraise card for an AltaPointe staff member.
What information is required on the card?
The person submitting an “AltaPraise” should include details of a specific, exceptional customer service performance by a particular staff member, exceeding the expectations set forth in the 5SCS Standards of Behavior. Incomplete AltaPraise cards/online forms will not be accepted.
What details should I include in the description of the customer service performance?
The description must identify how the staff member has demonstrated at least one of the “5SCS Employee Ideals.”
