Intellectual Disability Case Management Services
Case management supports individuals with intellectual disability by assisting with integration into the community. Case managers assess individual need, develop a care plan, advocate and link individuals to needed services, and regularly monitor each individual’s progress. To receive these services, consumers must meet the definition of intellectual disability as defined by the Alabama Department of Mental Health. |
