Assertive Community Treatment — ACT Team
ACT is a mobile, multi-disciplinary service delivery team providing treatment, rehabilitation and support services within a consumer’s home community. Consumers admitted to this program have had multiple hospitalizations. Services focus on symptom reduction and minimizing recurring acute episodes while enhancing the consumer’s quality of life. Services are available and accessible to enrolled AltaPointe consumers 24 hours per day/seven days per week in a manner and at locations conducive to consumer compliance. Staff members contact consumers at least once per week during the maintenance phase and up to several times per day when clinically required. |
